Virginia Advance Directives
In an emergency situation, it is very important to have key information immediately available. But you may not always have your Advance Directive available. The Crisis Plan Card lets you put this important information on a small form that you can carry around with you all the time (for example, in your wallet or purse).
With this card, you can write down the contact information for your doctors and agent, the location of your full Advance Directive, and vital medical information (allergies). The Crisis Plan Card on this website is different from other similar cards because this one functions as a legally-valid Advance Directive, allowing you to give legal authority to your agent.
